Tbf, he had a horrible time with covid right before the election in 2020. Like he legitimately almost died. They had to fly his huge dumper to the army hospital and pump him full of experimental drugs (at the time <one year deep into covid-19). If he wasn’t the president of the US, just some 75 year old in the Ozark mountains or something, he probably would’ve gone way too far along and died at home in bed drowned in his own mucus or just barely made it to an ER only to die within hours of having a tube shoved down his throat.

I think it’s something that would be traumatic regardless, but he has never truly been in danger before, and he knows he’s not getting any younger – The IDEA that it really could happen to him Any Fucking Time is probably totally foreign and utterly terrifying.
Adding on that he’s retraumatizing himself with the video repeatedly is a recipe for psychological disaster, which couldn’t happen to a nicer person. Hopefully it’s enough to keep him out of office, because The IDEA that someone with that level of unaddressed damage being in charge is pretty fuckin scary.
